This privacy policy is to provide you with information on how we use, share and protect your information including your health information.

Whole Voice Wellness is committed to comply with the Australian Privacy Act 1988. 


The reasons we need to collect personal information


We collect and store information in order to provide a service to you, or your child. We will only collect information that is required to provide therapy, write reports, or for business related purposes such as collecting fees for our services.


What kinds of personal information we collect and store


The information that we will ask you for, and store includes:


  • name, address and contact information 
  • Medicare, NDIS and/or health insurance details
  • previous assessments and treatments, medical conditions and treatment, allergies, and other records relating to providing services including school reports
  • financial records related to the service provided and fees charged; and
  • notes on consultations correspondence between you and the clinic, or other related service providers and the clinic
  • we may request a copy of your NDIS plan where applicable, or the required information from your plan to allow us to correctly invoice for our services, and to align our services with your stated goals



How we collect personal information


We will collect information through 

  • our website contact form
  • our registration form
  • email correspondence
  • sms messages and calls
  • collecting reports from other therapists, teachers, and medical professionals
  • stakeholder meetings with others involved in your care
  • during sessions



How your information is stored


All personal details and clinical information are stored in our patient management system.

Your/your child’s name, contact details, NDIS participant number and fees are also stored in our financial system.

Microsoft Office products are used in planning therapy and reports.

Other professional clinical assessment products are used to perform assessments and track progress.



How we'll use and disclose personal information


We primarily collect your information in order to provide a service to you or your child. Only a small number of people in our team will be able to access your information as required for business purposes. 

We will never sell your information to any other entity. We will only share your information with your explicit consent or:


  • where required in order to receive payment for our services
  • where required for therapy provision such as communication with your service co-ordinator or medical doctor
  • where disclosure is for the purpose of lessening or preventing a serious threat to life, health or safety and it is not suitable to gain consent 
  • where it is required by law or is ordered by a court or other legal entity



Information stored outside Australia


Where information is stored outside Australia, Choice Allied Health will ensure that storage is compliant with the Australian Privacy Act. Our primary information storage - our patient management system - stores all information within Australia.



Complaints


If you have a complaint about how we have collected or used your personal data, please email using the details on our contact us page. We will make every effort to respond to your complaint within a reasonable time.

If we have not responded within 30 days or you are not happy with our response, you are able to lodge a complaint with the Office of the Australian Information Commissioner at this site Office of the information commissioner
